Fire Investigations
Fire Prevention and Life Safety Officers will be called to the scene of fire in which: (5)
- loss exceeds $100,000
- the DC is unable to confirm the cause
- requires the securing of evidence
- requires the attendance of an investigator from the OFMEM
- the cause of the fire has been established as the result of a malfunctioned device or equipment

Witness statement forms will be completed by personnel attending the incident where there is a fire investigation conducted by:
- OFMEM Fire investigator
- Police
- Fire Inspector (when requested)

The Fire Chief or his designate shall be notified of all fires where: (4)
- losses will exceed $150,000
- all fire related deaths
- all explosions with significant damage
- any incident where there may be criticism directed towards MFES
Fire Investigations
An investigator from the Office of the Fire Marshal and Emergency Management (OFMEM) will be notified for:
- Fires resulting in a fatality or serious injury
- Explosions (where the explosion is the primary event)
- All fires in vulnerable occupancies
- Fire suspected of being incendiary
- Large loss fires, $500,000 and over or where the loss is significant to the community
- Fires of unusual origin or circumstance
- Fires resulting in unusual fire/smoke spread
- Fires involving circumstances that may result in widespread public concern
- Fires in multi-unit residential occupancies where fire spread is beyond unit of origin or where suspected Fire Code violations have impacted the event
- Fires involving clandestine drug operations or marijuana grow operations
Fire Investigations
The Officer will immediately notify the Fire Prevention and Life Safety Section when ________ or ___________ fire protection systems are discovered or when any Ontario Fire Code or Municipal by law __________ is encountered
- disabled
- inoperative
- infraction

It is not necessary to notify the OFMEM Fire investigator of the following fires:
A. Non fatal vehicle or boat fires
B. Single event, minor vandal arsons

- Chimney Fire reports (manufactured chimneys only)
- and all other applicable reports
