At the scene of a critical injury CO may need to provide:

• Name of injured person’s employer
• telephone number of employer
• accident location (municipal address)
• Employer name (and address if different from accident location)
• employer’s telephone number
• employer’s representative (and telephone number if different)
• injured person’s name
• injured person’s DOB
• nature of injuries and mechanics of injury if known

Who shall the CO notify that a workplace critical injury or death has occurred?

• DC
• if police officer is on scene, confirm that MOL has been notified, if not satisfied that MOL has been notified request that Comms contact MOL
