- White copy goes to Admin via inter office mail for VSA and/or symptom assist
- Yellow copy goes to the medical training officer via inter office mail when symptom assist is provided
- When MFES pads are attached, email the Zoll data file and a scanned copy of MAR form to VSA.training
- Extra copies of the MAR form will be disposed of
