Functional independence measure - level of pts disability and how much assistance required for individual to carry out ADL

0-7 (7 is highest)
Total score ranges from 18-126 (because there are 18 total tasks. We’re only responsible for a few)
Generally rated at admission and discharge
FIM score 7
Complete independence
Generally not given in hospital in pt
FIM score 6
Modified independence
Pt requires use of device, but no physical assistance
(Any AD, if in structured enviro and have not seen them do task outside, give them 6)
FIM score 5
Supervised or setup
Often head injury pts or stroke L hemi that are impulsive
FIM score 4
Minimal contact assistance
75% or more of task
(If you have to touch pt at all, automatically a 4 or lower. If you have to hover closely give a 4)
FIM score 3
Moderate assistance
50-74% of task
FIM score 2
Maximal assistance
25-49% of task
FIM score 1
Total assistance
Less than 25% of task OR requires more than one person to assist
FIM score 0

FIM what do you watch for when grading?
- Number of helpers

FIM walking qualifier
150 ft (50 m) for 7-3 50 ft (15 m) for 2-1
FIM walking - household exception
No helper - grade 5
Pt walks only short distances (minimum of 50 ft) INDEPENDENTLY w or w/out a device. The activity takes more tan a reasonable amount of time, or there are safety considerations.
Given for people who are independent but cannot walk long distances so they do not end up in nursing home. The pts goal must really be just to stay in house and live independently.
