ASIA
Define and describe the extent and severity of patien’s SCI,
Grade based on sensation and motor function
Most important part of ASIA
Talk to pt first! Ask them about extent of sensory and motor disturbances, where, etc
Number of dermatome for ASIA
28 assessed bilaterally
Sensory grading
0 = absent 1 = altered, decreased/impaired or hypersensitive 2 = normal NT = not tested
Number of muscle groups for myotome
10
C5 myotome
Elbow flexors - biceps
C6 myotome
Wrist extensors - radial extensor group
C7 myotome
Elbow extensors - triceps
C8 myotome
Finger flexors - along volar surface middle phalanx
T1 myotome
(Little) finger abduction (abd dig. Mini)
L2 myotome
Hip flexors
L3 myotome
Knee extensors
L4 myotome
Ankle dorsiflexors
L5 myotome
Long toe extensors
S1 myotome
Ankle plantar flexors
Nerves tested
C2- S4/S5
Muscle grades
0= total paralysis 1 = palpable/visible contraction 2 = active movement, full ROM w/ grav elim 3 = active move, full ROM against grav 4 = same as three AND mod restistance 5 = same as three AND full resistance
Determining sensor level
Most caudal level that is intact w/ both light touch and pin prince w/ everything else above testing normal
Determine motor level
Lowest key muscle function that has at least grade 3 w/ all muscle grades above getting a 5
Determine ASIA level
Determine sensory and motor separate then pick highest level between the two
ASIA grade A
Complete - no sensory or motor is preserved in sacral segments S4-5
ASIA grade B
Sensory incomplete.
Sensory but not motor function preserved below neuro level and includes sacral segment S4-5 —> no motor preserved more than 3 levels below the motor level on either side
ASIA grade C
Motor incomplete
Motor function preserved below neuro level, and more than 1/2 muscles < grade 3
ASIA grade D
Motor incomplete
Motor preserved below NLI and at least 1/2 muscles below >3
