What is this describing?
Benign tumours of the myometrium which regress after menopause due to reduction in sex hormones.

What are the risk factors for developing fibroids?
A
- Increasing age during reproductive years
- Black and Asian women
- Obesity
- Early menarche
- 1st degree relative

What percentage of fibroids are asymptomatic and when are they discovered?
A
- 50%

What are the signs and symptoms of fibroids?
A
- Menorrhagia - heavy and prolonged
- Anaemia
- Dysmenorrhoea - only if torsion/sacromatous change
- Subfertility - blockage/prevent implantation
- Mass effect - urinary symptoms, vein compression
- Increases obstetric malpresentation and can prevent head engagement.

How are fibroids investigated?
A
- Abdominal exam, bimanual exam
- TV USS
- FBC
- If fertility and issue - hysterosalpingogram/hysteroscopy

When do you treat fibroids?
A
When they are symptomatic

What is the medical treatment for fibroids?
A
- Tranexamic acid/NSAID 1st line but usually ineffective
2. GnRH to shrink fibroids for surgery

What is the surgical treatment for fibroids?
A
- Hysteroscopic for small fibroids (TCRF)
- Myomectomy if medical failed and fertility still desired
- Hysterectomy is the only cure
- Uterine artery embolisation avoids anaesthetic, extremely painful, affects fertility

What are the complications of fibroids?
A
- 0.1% are malignant
- Red degeneration in pregnancy - pain due to inadequate blood supply
- Torsion of pedunculated fibroids
