In what group of people is endometriosis more common?
A
Between 30-45 years, nulliparous women.

What is the underlying pathology involved in endometriosis?
A
- Oestrogen dependent, regresses after menopause and during pregnancy.
- Occurs throughout pelvis.
- Inflammation with progressive fibrosis and adhesions
- Probably due to retrograde menstruation

What is this a presentation of?
Cyclical pelvic pain, dysmenorrhoea prior to onset of menses, deep dyspareunia, subfertility, pain on passing stools, dysuria. Fixed retroverted uterus (advanced disease).

How is endometriosis investigated?
A
- Gold standard is laparoscopy and biopsy for histological confirmation
- TV USS
- Ovarian endometriotic cysts (sign of severity)

What are the differentials to consider in endometriosis?
A
- Adenomyosis
- Chronic pelvic inflammatory disease
- Chronic pelvic pain
- IBS

What is the medical management for endometriosis?
A
- NSAIDs +/- paracetamol +/- opioids
- COCP back to back
- IUS
- POP, GnRH analogues

What is the surgical management for endometriosis?
A
- Only once medical failed or if fertility desired
- Laparoscopic ablation/excision to destroy endometriosis
- Adhesiolysis
- Hysterectomy and bilateral salpingo-oophorectomy as last resort
