feeding & eating disorders
anorexia nervosa, bulimia nervosa, binge eating disorder, avoidant/restrictive food intake disorder, pica, rumination disorder
prevalence
anorexia nervosa- 0.5%, bulimia nervosa- 1-2%, binge eating disorder: 2-4% (M=F)
development theory
predisposing factors: female, genetics, societal emphasis on thinness; precipitating factors: stress of adolescence; perpetuating factors: dieting –> binge eating –> dieting
anorexia nervosa
restriction of intake causing low weight, intense fear of gaining weight, disturbance of self-body image
behavior: obsession with food, peculiar eating
physiology: hypothermia, hypotension, lanugo (soft hair), amenorrhea, osteoporosis (non-specific sx of starvation)
treatment for AN
behavioral intervention (CBT), NO DRUGS
outcome for AN
1/3 -1/2 fully recover; 5% per decade of follow-up (starvation & suicide), the rest= alive but still not recovered
bulimia nervosa
recurrent episodes of binge eating and inappropriate compensation to prevent weight gain (laxatives, vomiting), occurs 1+/wk for >3m
physiology: normal body weight, erosion of teeth, enlarged salivary glands, gastric rupture (rare)
treatment for BN
cognitive behavioral therapy (CBT), SSRIs
binge eating disorder
recurrent episodes of binge eating with distress; 1+/wk for >3 m, increased frequency of mood & anxiety disorders, M=F, onset middle-age; CBT, SSRIs
