major depressive episode
“People Who Are Depressed Cry And Feel So Sad”
>2 weeks
1) depressed mood OR anhedonia
2) 4 additional symptoms:
psychomotor worthless anhedonia depressed concentration appetite fatigue sleep (SPECIFIC - middle of the night awakening, early morning awakening) suicidal
epidemiology
onset in early 20s, lifetime prevalence 17% (women 20-25%; men 7-12%)
persistent depressive disorder/dysthymic disorder
1) “chronic depression”, depressed mood for >2 years, never w/o symptoms for 2 months
2) two additional symptoms:
"who, cry and feel so sad" low self-esteem (worthless) concentration appetite fatigue sleep hopelessness (suicide)
major depressive disorder
major depressive episode without mania/hypomania
subtypes: psychotic, melancholic, anxious, seasonal, atypical, peripartum, catatonia
increased risk with 1st degree relative, events (loss, trauma, stress)
persistent depressive disorder/dysthymic disorder

treatment
anti-depressants, termination of episode + 6 months following;
antipsychotics, ECT if depression with psychotic features
