psychosocial factors (gender-based violence, SES, caretaker role), reproductive events, biological vulnerability
F<M psychiatric diagnoses
substance abuse, antisocial personality disorder, narcissistic PD, OCPD
bipolar disorder in females
F>M in BP II, more mixed episodes, more depressive episodes, rapid cycling, later age of onset
schizophrenia
later age of onset (25-35), more benign course
pre-menstrual dysphoric disorder (PMDD)
3-8% of menstruating women, symptom onset @ luteal phase, mood > physical symptoms; depression/irritability, fatigue, hypersomnia
tx: lifestyle interventions, Chasteberry, SSRIs, Yaz OCP
depression during pregnancy
elevated estrogen during 1st-trimester; poor overall health, alcohol use, smoking increase risk; same prevalence as non-pregnant women
untreated depression and anxiety
increased risk pre-term delivery, preeclampsia, and low birth weight (excess cortisol)
treatment during pregnancy
psychotherapy, light therapy, limited psychotropic meds
post-partum baby blues
normal, <2w and non-impairing
post-partum psychosis
psychiatric emergency!!! 1-2/1000 women; high risk in bipolar patients, onset 24h- 3w post-partum, rapid mood swings, delusions, hallucinations, insomnia, high rate suicide and infanticide
post-partum depression
10-15% prevalence, normal depression symptoms, + OCD thoughts; tx with anti-depressants
perimenopause
1-5 years pre-menopause, increased risk of 1st MDE during transitional period, hot flashes, perspiration, low energy, poor concentration, change in weight, sleep, libido; tx with estrogen replacement for mild symptoms
