isolated dysfunction of facial nerve
typically presents with unilateral facial weakness
branches of facial nerve
temporal zygomatic buccal marginal mandibular cervical
facial nerve function - motor
muscles facial expression
stapedius of inner ear
facial nerve function - sensory
taste anterior 2/3 tongue
facial nerve function - parasympathetic
submandibular and sublingual salivary glands
lacrimal gland
UMN innervation of forehead
both sides of brain
LMN innervation of forehead
one side brain
UMN and the forehead
forehead will be spared
patient can move forehead on affected side
LMN and the forehead
forehead will not be spared
unable to move forehead on affected side
UMN causes - unilateral
CVA - stroke
tumours
UMN causes - bilateral
pseudobulbar palsies
MND
bells palsy
idiopathic
unilateral LMN facial nerve palsy
bells palsy - prognosis
majority fully recover over several weeks
1/3 left with residual weakness
bells palsy Mx
if present within 72hrs can give prednisolone
-50mg for 10 days
or
-60mg for 5 days, then 10mg 5 days
also, lubricating eyedrops
bells palsy Mx if develop eye pain
opthalmoglogy review for exposure karatopathy
