exam 1: Gall bladder disorders Flashcards
normal physiology
fat intake-> release of cholecystokinin-> gall bladder contracts and sphincter of Odii relaxes-> bile into duodemum-> fat metabolized
gall bladder’spurpose
store and concentrate bile
cholelithiasis
gallstone formation in gallbladder
risk factors for cholelithiasis
- sedentary life
- obesity
- increased fat diet over years
path chain of cholelithiasis
increased cholesterol in bile-> cholesterol crystals aggregate->stone growth
clinical mani of cholelithiasis
- abdominal pain-RUQ (biliary colic)
- jaundice if stone is in CBD
- heart burn/indigestion/flatulance
- intolerance of fatty foods
diagnosis of cholelithiasis
history and physical exam
ultrasound
endoscopic retrograde cholangiopancreatography
cholecystitis
inflammation of the gallbladder wall or cystic duct
etiology of cholecystitis
usually gallstone obstruction of cystic duct or CBD
patho of cholecystisis
obstruction causes the gall bladder to become distended and inflamed due to increased pressure and ischemia
clinical mani of cholecystitis
pain RUQ
fever
increased WBC
abdominal guarding (increased pain with palpation– Murphy’s sign)
