emergency contraception - 3+1 options, in order of worst to best efficacy
levonorgestrel (LNG) pill 1.5mg PO Stat ($1-25)
- ghetto solution - take 25 tabs LNG 30microg (microlut), twice over 12 hours, so 50 tabs total
ulipristal (UPA) pill ($40-50)
copper IUD insertion ($90-100+cost of insertion)
emergency contraception - options if pre-ovulation
LNG, UPA, and copper IUD are all options
emergency contraception - options if post ovulation
copper IUD only
emergency contraception - when is the latest after unprotected sex that it can be initiated
* consider contraception options
copper IUD or UPA - 120 hours (5 days)
LNG - 72 hours
emergency contraception - options in high BMI patients
copper IUD or UPA
starting contraception after emergency contraception - when can you do it, when will it take effect
LNG - start immediately, COCP effective immediately but the rest is standard wait
UPA - 5 days after administration, will be effective at standard wait for effectiveness (implanon will work straight away tho)
copper IUD works immediately
contraindications for emergency contraception (6)
- some apply to specific types of EC
all emergency contraception - pregnancy
copper IUD - infection or distortion
LNG, UPA - allergies, liver disease, on liver enzyme inducers
emergency contraception while on liver enzyme inducers - options
copper IUD is best
take double dose of LNG (3mg)
emergency contraception if you have diarrhea, or GI malabsorption
copper IUD
oral emergency contraception - common adverse effects (5)
headache nausea vomiting dysmenorrhoea altered bleeding pattern
copper IUD contraception - adverse outcomes to consider
altered bleeding pattern
perforation, infection, expulsion
emergency oral contraception - patient had a vomit afterwards. what interval requires repeat dose
LNG - if vomits in <2/24
UPA - if vomits in <3/24
