What is the key difference between binge eating disorder and bulimia?
BED , there is no compensatory behavior (purging, fasting, exercise, enema)
Cognitive narrowing in starvation
Anorexic pts have problems with attention/concentration, cognitive acuity, and decision-making (leads to vicious cycle of bad decisions).
Overall, what meds are helpful in anorexia?
None. Focus more on CBT, hospitalize involuntarily if necessary.
How many pounds per week of weight gain should one go for?
1 lb per week
Is med treatment helpful in bulimia?
Yes, it is effective in reducing core symptoms of binge eating and purging.
DBT and bulimia
Can also be helpful.
Which SSRI is shown to reduce binge frequency? (ie, most studied?)
sertraline
What things can trigger pica?
Iron-deficiency anemia and malnutrition
What is rumination disorder?
Repeated regurgitation of food over at least 1 month. Regurgitated food may be rechewed, reswallowed, or spit out. Not due to GI or other medical condition.
Primary Nocturnal Enuresis vs Secondary
Primary: Never achieved satisfactory nighgttiem dryness.
Secondary: Wetting restarted after 6 month period of dryness.
What age must one be to get the primary or secondary enuresis diagnosis?
5, because that is the typical age for the bladder to mature.
What medication to be used for nocturnal enuresis?
Desmopressin Acetate (intranasal spray has increased risk of water intoxication adn hyponatremic seizures)
Also Imipramine because TCA’s decrease time spent in REM sleep, it stimulates vasopressin secretion, and relaxes the detrusor muscle.
Encopresis
Repeated passage of feces into inappropirate fplaces (clothes, floor) whether INVOLUNTARY or INTENTIONAL (could be either) . Must be at least 4 years old.
