what is the problem with phlorizin?
A
- very poorly absorbed orally
- non specific so inhibits SGLT1 as well resulting in osmotic diarrhoea

what do SGLT2 inhibiotors do?
A
-they inhibit SGLT2 (sodium glucose transport proteins) in the kidneys, decreasing the amount of sugar uptake by one quarter and meaning that one quarter is just peed out

what are some direct effects of SGLT2i?
A
- causes renal protection
- weight loss
- diuresis
- improved myocardial energetics

what are some side effects of SGLT2i?
A
- thrush!! (you’re making them pee out sugar)
- fournier gangrene
- hypovolemia and hypotension
- diabetic ketoacidosis

when would you take a SGLT2i ?
A
- high CVD, CKD or heart failure and second line to metformin
- NOT high CVD, CKD or heart failure but patient need another drug after metformin (second line to metformin) and are worried about hypoglycaemia
- NOT high CVD, CKD or heart failure but patient need another drug after metformin (second line to metformin) and need to lose weight
