Chronic high level of glucose passing through the glomerulus causes scarring

CKD
What is glomerulosclerosis?
Scarring due to the high levels of glucose passing through the glomerulus
What is a key feature of diabetic nephropathy?
Proteinuria
Why is proteinuria a key feature of diabetic nephropathy?
Damage to the glomerulus allows protein to be filtered from blood to urine
What should all patients with diabetes have done annually?
Screening using urinary albumin:creatinine ratio (ACR)
What ACR would suggest microalbuminuria?
ACR > 2.5
When should the urine sample be taken to test for microalbuminuria?
Early in the morning
What is dietary management of diabetic nephropathy?
Limit dietary protein
What should be tightly controlled in someone with diabteic nephropathy?
Glycaemic control
Blood pressure
What should the blood pressure be maintained at in someone with diabetic nephropathy?
<130/80mmHg
What is the medical management of diabetic nephropathy?
ACE inhibitor or angiotensin-II receptor antagonist
What is the medical management of choice in someone with diabetic nephropathy?
ACEi
When should ACEi be started in someone with diabteic nephropathy?
Urinary ACR of 3 mg/mmol or more
