Isotretinoin

  • Indication: Severe recalcitrant nodular cystic acne. Reserve for pt who are unresponsive to conventional therapy, including abx
  • Preg Cat: X

iPLEDGE

  • 2 forms of contraceptive starting 1 mo b/4, during, and after therapy
  • 2 negative urine/blood preg tests
  • Each mo, the prescriber enters the preg tests results and the 2 forms of contraception
  • The pharmacist must obtain authorization from the iPLEDGE system prior to dispensing for both male and female pts

Must registered:

  • Wholesalers
  • Prescribers
  • Patients
  • Pharmacies
  • Filled and Pickup winder: 7 days
  • Supply: 1 mo (30 days supply). No refills. Need new rx each month
  • Forms: 10, 20, 30, 40mg soft gel cap
  • Dose: > 12 y.o 0.5-2mg/kg/d divided BID x 15-20wks with FOODs (high fat meals)
  • Storage: protect from light
  • Every visit: CBC, LFT, preg test

SEs

  • Dry skin, Cheilitis
  • Sun sensitivity
  • Psych: suicidal, depression, psychosis, violence
  • Hepatotoxicity
  • Bone marrow suppressants

Lab monitoring

  • LFTs
  • CBC
  • Cholesterols

DDI: avoid using

  • Tetracyclines: Inc risk of preg
  • Progesterone only pills: not enough for contraception
  • St. John’s Wort: inc risk of preg
  • Long term corticosteroids or phenytoin: Increase risk of bone loss
