Methylphenidate

  • Ritalin IR: BID-TID b/4 meal. Max 20mg TID. Rapid abs
  • Ritalin LA: Can open n sprinkled on applesauce
  • Methylin: chewable tabs n oral sol’n
  • Concerta OROS: osmotic system
    Come in: 18, 27, 36, 54 mg ER
  • Metadate CD: 30% IR n 70% ER
  • MOA: Increase dopamine and NE. Stimulants
  • SEs: delayed onset of sleep, HA, decreased appetite = wt loss, slow growth, may exacerbate bipolar or psychotic condition. Heart problems pt, see cardiologist before.
    Tics (most common with patch)

DDIs:

  • MOA inhibitors
  • Foods high in tyramine content
  • TCAs
  • Antihypertensive agents
  • Sympathomimetics
  • Warfarin
  • Diamox: inc amphetamine [ ]

DOC

  • School age children: amphetamine/methylphenidate
  • Atomoxetine: use if fx object to tx with controlled sub
  • Alpha-2 adrenergic: helpful when used combined
