torniquet test/ rumpel leads/ capillary fragility test
how they do the torniquet test??
- get the BP of your patient
- inflate the BP cuff midway between the SBP and DBP
- inflate the BP - 100mmHg and let stay there for 2-3 minutes
- deflate cuff and make an imaginary box about 1 inch square below the antecubital fossa
- count the number of the petichae (rashes)
how is it determined if it is positive for tornique test?
if the petichae count is 10-20 within the imaginary box
stages of DHF
a. febrile or invasive stage ( last for 1-3 days)
b. toxic/ hemorrhagic stage (4-7 days)
c. convalescence/ recovery stage (8-10 days)
s/sx of febrile stage in DHF
- rash (appears at the 3rd day)
- fever (40C or 104F) / chills
- arthralgia / malaise
- n/v, loss of appetite
- headache / abdominal pain
s/sx of toxic or hemorrhagic stage
- epistaxis
- gum bleeding
- melena
*** avoid dark color beverages — masks s/sx of bleeding - narrow pulse pressure ( hypotension, weak&thready pulse)
convalescence/ recovery stage
- regains appetite
- BP normalizes
- generalized flushing
