Nursing Process
a systematic method of problem solving that provides the nurse with a logical, organized framework from which to deliver nursing care. The foundation for decision making for a nurse. Includes: 1) Assessment 2) Planning/diagnosing 3) Implementation 4) Evaluation APIE!!
Assessment
The first stage which involves collection of pt data through pt history and physical assessment as well as MSE and psychosocial assessment.
Planning/Implementation
Determine needs and goals. Implement nursing interventions. Working through feelings and behaviours.
Evaluation
The final stage where pt’s are better able to manage their needs and cope with their problems. The nurse and pt reflect together on what they have accomplished. This is a time to celebrate success and where the nurse-pt relationship is terminated.
