Pt has facial weakness following IAN - what has likely happened?
Pt has developed a palsy of the motor part of the facial nerve
How can facial nerve palsy w/ IAN occur?
If IAN not administered correctly - needle advanced too far posterior, result in LA permeating the facial nerve within the parotid gland
When would likely see facial palsy following incorrectly administered IAN?
Usually presents within mins but can be delayed
What are the terminal branches of the facial?
Temporal, zygomatic, buccal, mandibular, cervical
Where does the facial nerve split into terminal branches?
Parotid
What does temporal branch of facial nerve supply?
Frontalis and orbiculares oculi
What does zygomatic branch of facial nerve supply?
orbiuclaris oclui
What does buccal branch of facial nerve supply?
Buccinator and orbiculares oculi
What dose mandibular branch of facial nerve supply?
Muscles of lower lip and chin
What does cervical branch of facial nerve supply?
Platysma
How would assess for damage to facial nerve?
Temporal - raise eyebrow
Zygomatic - close eyes
Buccal - puff cheeks
Mandibular - smile
What is facial nerve?
Motor nerve - supply muscle of facial expression
How to manage this as dentist?
Reassure pt likely temporary and will wear off with anaesthetic
If struggling close eye needs eye patch, if unable to close needs eye drops to lubricate
Review next day to ensure worn off
What if function of trigeminal nerve?
Provided both motor and sensory function via 3 main branches - ophthalmic, maxillary and mandibular
Which branch of the trigeminal nerve is motor?
Mandibular branch to MoM
How would assess trigeminal nerve?
Test sensation - apply light touch
Motor - clench teeth and palpate massester
What cranial nerve supplies taste to tongue?
Facial nerve via corda tympani (anterior 2/3rd)
Glossopharyngeal (posterior 2/3rd)
If pt has reduced movement of tongue what nerve is likely to be affected?
Hypoglossal or vagus nerve
Hypoglossal - motor to all expect palatoglossus
What will damage of the hypoglossal result in?
Deviaton of tongue to affected side
