SCC - accounts 90% cancer in oral cavity
What are RFs for SCC?
Tobacco
Alcohol
Betel quid
HPV
What type of HPV is associated w/ SCC?
HPV16
What are suspicious features of ulcer that suggest possible malignancy?
White or red leison
Ulcer w/ rolled margin
Induration
Altered sensation in region
Give examples of lesions that would require a 2 weeks referral
Unexplained non-healing ulcer present > 3 weeks
Persistent and unexplained lump in neck
Red or white/ red patch
Lump in lip or oral cavity
Prior to carrying out biopsy, how would pt MH affect approach
General MH increase bleeding risk - liver disease
Impaired healing - diabetes
Drugs - steroids/ anticoagulants
What are the two types of biopsy?
Incisional
Excisional
What is an incisional biopsy?
Partial removal or lesion - representative area inc normal tissue to adequate depth
What is excision biopsy?
Lesion removed in entirety
When is excision biopsy appropriate?
Non-malignant lesion
What is adv and disadv of punch biopsy compared to scalpel?
Adv - simple and quick to carry out
Disadv - not always a large/ representative enough sample, harder to appose
What techniques can be used to ensure biopsy is representative enough?
Inclusion of non-affected area
Careful handling - prevent crushing sampl
Full thickness epithelium
Adequate size
What history is important to provide to pathologist?
History of lesion - length of time, evolution, symptoms
Charactersiitics - size, shape, consistency
Clinical improvement
