10-year old boy has avulsed tooth. What information want to know over phone prior to seeing patient?
Timing - length of time tooth has been avulsed
Location avulsion
Tooth recovered
Any inhalation risk
Any other injuries - should pt go to A+E
What instructions give over phone?
- Pick tooth up by crown (not pointy root)
- If dirty, wash 10 seconds under cold water to remove debris
- Encourage re-implantation
What advise to give if don’t want to reimplant tooth?
Tooth placed in medium - milk/ contact lenses solution
Or
Store tooth inside cheek/ spit into cup and store in saliva
DONT STORE IN TAP WATER
What hx to take if dental trauma?
Where
How did it happen
Any LOC/ vomit/ dizziness
Any other injuires
IO exam
EO exam
Why is it important to ask about vaccination?
Important to know about tatnus status
If not up to date advise should attend for tetanus booster
What is guidelines for re-implanting closed apex avulsed tooth with dry time less 60 mins?
Use wet gauze gently remove debris
Administer LA
Irigate socket with saline
Reimplant tooth
Verify position clinically and radiographucally
Split tooth
How long should splint be placed avulsed tooth?
2 weeks
What else consider as part of mx plan for avulsed tooth?
Oral ab for 7 days
What tx needs to be provided if closed apex tooth avulsed?
RCT prior to re-implantation or 7-10 days post implantation
When is RADs recommended for review avulsed tooth?
Post-splint removal
3 months, 6 months and 1 year
What are risks w/ reimplanting tooth?
Root resorption
Ankylosis
Loss vitality
What instructions give to parents post preimplantation?
Tooth may discolour
Tooth may become non-vital
To contact if any signs of infection
Avoid contact sports till splint removed
Maintain OHE
