permanent distal obstruction/destruction and enlargement of airspace, no fibrosis
chronic bronchitis
excess mucus, cough/sputem for 3 month/2 + year, airflow obstruction, rule out chronic cough, reversible early

host factors
BHR, BDP, genetic, maternal smoke, child asthma
mech of copd
chronic inflam, imbalance of elastase/anti elastase, oxidative stress
Clin presentation
dysnnea/tachy, wheeze, increased expiration, cough/sputem, WL, hoover, TA paradox
small airway obstruction
more goblet cells, less elastase, more fibrosis, thick airway, less max exp flow
less expiratory flow
less recoil, more airway R, less radial traction
hyperinflation
elastic increase WOB, air trap, exp issue but feels hared to INSPIRE
hypoxemia cause
usually V/Q, periferal airway destruction, more headspace.
