Q

DKA

Symptoms more related to T1 or T2?

A

T1

No insulin/not enough–glucose goes sky high–PPP–fat breakdown–Kussmaul respirations–Metabolic acidosis–LOC decreases

Q

DKA treatment

  • Find the ___
  • How often for BS and K levels?
  • Give what type of insulin?
  • Need ECG?
  • Hourly what?
  • Check ___
  • Give ____
A
  • Find cause
  • Blood sugar and K levels hourly
  • Give IV insulin
  • Yes need ECG
  • Hourly outputs
  • Check ABGs
  • Give IV fluids

Q

DKA tx

What does IV insulin do?

A

Decreases glucose and K by driving them out of vasc. space and into the cell

Q

DKA tx

Why give IV fluids?
How to give IV fluids?

A

Polyuria (peeing a lot) can lead to shock

Start with NS then when BS goes down to about 250-300 switch to D5W to prevent hypoglycemia. **doc may add K at some point

16
Q

Vascular problems: micro and macro

Sugar irritates vessel lining —accumulation of sugar does what?

A

Decrease size of vessel lumen therefore decreasing blood flow

17
Q

Vascular problems: micro and macro

What 2 problems?

A
Diabetic retinopathy (can go blind!!!)
Nephropathy (can lead to dialysis!!!)
18
Q

Neuropathy

What 4 big problems?

A

Sex problems
Foot/leg problems
Neurogenic bladder
Gastroparesis

19
Q

Neuropathy

What sexual problems?

A

Impotence

Decreased sensation *sugar kills nerves!

20
Q

Neuropathy

What foot/leg problems?

A

Pain
Paresthesia
Numbness

Teach foot care!!!!

21
Q

Neuropathy

What is neurogenic bladder?

A

Bladder does not empty properly

22
Q

Neuropathy-Neurogenic bladder

The bladder may empty spontaneously (____) or it may not empty at all (____)

A

Incontinence

Retention

23
Q

Neuropathy

Gastroparesis is what?

A

Stomach emptying delayed so increased risk for aspiraion

24
Q

Increased risk for _____

A

Infection!!!

*Diabetes alters immune system
