A
A - age/appearance (treatment can vary depending on age)
S - self/someone else (who is it for? who is speaking?)
M - medication (existing conditions? prescription or OTC?)
E - extra medication (have they tried anything for this before?)
T - time persisting (duration of symptoms)
H - history (first time or reoccurring? medical history?)
O - other symptoms (may not appear to be related)
D - danger symptoms (what do the symptoms indicate as the cause?)

W - who is the patient? W - what are the symptoms? H - how long have the symptoms been present? A - action taken? M - medication being/been taken?
