What is coeliac?
Autoimmune condition causing sensitivity to Gladin fraction of gluten via tissue transglutaminase
What is the histology in coeliac?
Partial or subtotal villous atrophy
Increase in intraepithelial lymphocytes
What are the signs and symptoms of coeliac?
Weight loss Diarrhoea and/or constipation Abdo pain Blating Anaemia Fatigue Dermatitis herpetiformis
What is dermatitis herpetiformis?
Intensely itchy skin condition, usually symmetrical on extensor surfaces
How is coeliac diagnosed?
Coeliac serology- anti tissue transglutaminase, IgA
HLA status- + in 97% coeliacs
Distal duodenal biopsy for villous atrophy
How is coeliac managed?
Withdraw gluten and refer to state registered dietician
What is refractory coeliac?
Doesn’t respond to withdrawal of gluten
How is refractory coeliac managed?
Steroids
What are the complications of coeliac?
Refractory coeliac Increased risk of -Oesophagel carcinoma -Small bowel adenocarcinoma/lymphoma -Bowel cancer
