Infection of biliary tree
What causes cholangitis?
Obstruction –> stasis and infection
What are the clinical features of cholangitis?
Charcot triad- RUQ pain
fever
Jaundice
What investigations are done for cholangitis?
Bloods- culture, U&Es, elevated WCC, CRP and LFTs
ERCP
US
What is the management of cholagitis?
Resus
IV antibiotics
Biliary depression, operative or non op
What is a complication of cholangitis?
Acute pancreatitis
What is primary biliary cholangitis?
Progressive autoimmune inflammation and destruction of small bile ducts
What is PBC associated with?
Other autoimmune conditions
Middle aged women
What are the clinical features of PBC?
Asymptomatic
Cholestatic jaundice
Pruritis
Hepatosplenomegaly
How is PBC diagnosed?
Anti mitochondrial antibodies
Raised IgM
What is the management of PBC?
Symptomatic
Liver transplant
UDCA
What is primary sclerosing cholangitis?
Obliterative inflammation and fibrosis of biliary tree
What is PSC associated with?
UC
What is the presentation of PSC?
Recurrent cholangitis
Fatigue
Cholestatic jaundice and pruritis
How is PSC diagnosed?
ERCP/MRCP
Raised ALP, GGT and bilirubin
