What is chronic liver disease?
Liver disease persisting >6 months
What are the causes of chronic liver disease?
Chronic hepatitis
Chronic cholestasis
Fibrosis and cirrhosis
Liver tumours
How does compensated chronic liver disease present?
Asymptomatic, detected by abnormal LFTs
How does decompensated chronic liver disease present?
Gynaecomastia, spider navei, clubbing, palmar erythema Jaundice Ascites Varices Encepahlopathy
What are the sequelae of chronic liver disease?
Ascites
VArices
Encephalopathy
What is ascites?
Accumulation of fluid in peritoneal cavity
What are the features of ascites?
Abdo distension
Shifting dullness
What investigations are done for ascites?
Serum ascites albumin gradient
Protein and albumin concentration
Cell count
Bloods- FBC, LFTs, U&Es
What can serum ascites albumin gradient suggest?
Elevated i.e.. >11 suggests cirrhosis
Low suggests cancer
What is the management of ascites?
Diuretics e.g. spironolactone
Large volume paracentesis with albumin cover
Why is albumin cover indicated in large volume paracentesis?
Prevent circulatory dysfunction
What does recurrent ascites indicate?
Need for liver transplant
What are varices?
Abnormally dilated vessels with tortuous course
What causes varices?
Portal hypertension
Where do varies occur?
Porto-systemic anastomoses- skin, oesophagus, stomach, rectum, posterior abdominal wall
What is the management of varices?
Endoscopic band ligation
Beta blockers
What is the emergent management of bleeding varies?
Resus
Terlipressin
Emergency endoscopy
What is hepatic encephalopathy and what causes it?
Failure of liver to break down ammonia, which crosses the blood brain barrier and causes confusion
What triggers hepatic encephalopathy?
GI bleeds Infection Constipation Dehydration Drugs
What is the presentation of hepatic encephalopathy?
Confusion
Altered GCS
Apraxia
Flapping tremor
What is the management of hepatic encephalopathy?
Lactulose
Treat underlying cause
What does recurrent hepatic encephalopathy indicate?
Need for transplant
What is cirrhosis?
End stage chronic liver disease
Advanced fibrosis replaces functional parenchyma, preventing function and blood flow into and out of liver
What are the features of cirrhosis?
Jaundice haematemesis Melaema Ascites Pruritis Oedema
What are the complications of cirrhosis?
Liver failure
Portal hypertension
Risk of hepatocellular carcinoma
