what is chorioamnitis?
inflammation of the amniochorionic (fetal) membranes of the placenta, typically in response to microbial invasion
cause- chorioamnitis?
96% ascending polymicrobial infection
-E.coli
-mycoplasma
-anaerobes
-group B strep
risk factors- chorioamnitis?
anything that introduces microbes into the uterus
-invasive pre natal testing
-prolongued rupture of membranes
-prolonged labour
-repeated digital exams
-nullparity
-meconium stained liquor
chorioamnitis- presentation?
-offensive PV loss
-maternal pyrexia
-abdominal pain
-fetal CTG concerns
what temperature should a mother have to suspect chorioamnitis?
suspect it at 39 degrees
how is chorioamnitis diagnosed?
Diagnosis typically not confirmed until after delivery when sample of fetal membranes is taken or through amniotic testing
treatment- chorioamnitis?
Broad-spectrum antibiotics
-Delivery - if not in established labour needs IOL or LUSCS (C section)
-Subsequent increase in risk of post-partum haemorrhage - reduce with active 3rd stage syntocinon infusion
- Avoid post-partum intra-uterine contraception (PPIUC)
