Hemoconcentration
An increase in the ration of formed elements to plasma caused by leaving the tourniquet on too long or too tight
* Can alter some test results
hemolysis
can occur if the tourniquet is too tight or left on too long
destruction of rbcs
can alter some test results
petechiae
small red spots on the skin
caused by tourniquet that is too tight
hematoma
reddened, swollen area where blood collects under the skin
forms when the extra pressure from the tourniquet forces blood out through the puncture

Patient’s name and DOB
* Inpatient : hospital MRN, Room and bed number
* Name or Code of Dr making the order
* Tests requested
* Test status (STAT, Timed, fasting
pt. identifcation
2 identifiers,( arm band , verbal )
* Guidelines set by The Joint Commission
* Reduce the risk of misidentifying patients
