How do nitrates work to decrease cardiac work load?
Nitrates decrease venous tone.
Contraindications with Nitroglycerin
- patient with Right ventricular myocardial infarction, shock, and cardiac tamponade.
- should NOT be used with severe anemia, cardiomyopathy and increased ICP.
Interactions with Nitroglycerin
- patients taking phosphodiesterase-5 inhibitors within 24-48 hrs (sildenafil).
- herbal supplements can increase hypotensive effects.
Titration Control
Follow titration control when giving IV Nitroglycerin to ensure safety. This control is a safe adjustment based on patient’s response to medication.
Nursing teaching for patient taking Nitro
- notify MD if chest pain is not alleviated with 3 Nitroglycerin tablets.
- avoid situations or activities that can further vasodilate the body or cause hypotension.
- change positions slowly
