A Property Damage or Loss Report is completed and submitted through CoC. With exception of vehicles.

Who is responsible for facilitating the replacement of equipment?

District Chiefs or Division heads.

What happens to damaged PPE?

Collected by DC and turned over to health and safety captain.

What happens if the damaged or lost property is greater than $500?

The accident work group reviews the incident, identifies causes, confirms $ loss, and makes recommendations for training/corrective action

Question 5

Who reviews the Property Damage or Loss Report?

Answer 5

Deputy Chief of Administration and they maintain an archive of the reports. HR will file the reports in the employees personal file.

Who follows up with the corrective action?

Answer 6
