Notify direct supervisor and SAM2 ASAP. Complete a firefighter casualty report. Notify wellness/workers comp director by telephone.

Complete first report of injury form and the physician’s report submit to wellness/workers comp director.

Who performs medical treatment on injured members?

One of the districts medical providers unless an emergency exists then take member to closest appropriate facility. Contact wellness/workers comp director for assistance.

Can workers comp claim be denied?

Yes they can be approved or denied.

Who is liable for costs from a workers comp claim?

If claim is approved, member is not liable, if denied, all costs are on member.
