Separation Anxiety Disorder
Developmentally inappropriate excessive fear and anxiety concerning separation from attachment figures
Persistent in experience and duration, lasting at least 4 weeks in children/adolescents and at least 6 months in adults
Evidenced by at least 3 of following symptoms:
- recurrent excessive distress when anticipating or experiencing separation from home or major attachment figures
- persistent and excessive worry about losing major attachment figures
- persistent and excessive worry about experiencing event that causes separation
- persistent and excessive reluctance or refusal to go anywhere
- persistent and excessive fear of being alone
- persistent and excessive reluctance or refusal to sleep away from home or figures
- repeated nightmares involving theme of separation
- repeated complaints of physical symptoms when separated from major attachment figures
Selective Mutism
Consistent failure to speak in specific social situations in which there is an expectation for speaking despite speaking in other situations
Interferes with educational and occupation achievement
Duration is at least 1 month
Specific Phobia
Fear or anxiety about a specific object or situation
(specifier of Phobic Stimulus is tagged on to diagnosis)
Phobic stimulus almost always evokes immediate fear/anxiety and is actively avoided
Fear and anxiety is out of proportion to actual danger posed, is persistent, causes significant distress or impairment, duration typically lasting for 6 or more months
Social Anxiety Disorder
Fear or anxiety about 1 or more social situations in which the individual is exposed to possible scrutiny by others
- social interactions, being observed, performing in front of others
[must occur in peer settings for children]
Fear of acting in a way or showing anxiety symptoms that will be negatively evaluated
- fear of humiliation, embarrassment, rejection
Social situations almost always provoke fear or anxiety and are avoided or injured with extreme anxiety
Fear or anxiety is out of proportion to actual threat posed by social situation
Persistent, typically lasting 6 months or more
Panic Disorder
Recurrent unexpected panic attacks
Panic attack is an abrupt surge of intense fear or intense discomfort that reaches a peak within minutes
1 or more of the following symptoms occur:
- palpitations, pounding heart, accelerated heart rate
- sweating, trembling, or shaking
- sensations of shortness of breath or smothering
- feelings of choking
- chest pain or discomfort
- nausea or abdominal distress
- feeling dizzy, unsteady, light headed, or faint
- chills or heat sensations
- paresthesias (numbness or tingling sensations)
- derealization or depersonalization
- fear of losing control or “going crazy”
- fear of dying
At least 1 of the attacks has been followed by 1 month (or more) of one or both of the following:
- persistent concern or worry about additional panic attacks or their consequences
- a significant maladaptive change in behavior related to the attacks (to avoid attacks such as avoidance of anxiety provoking situations or unfamiliar situations)
Panic Attack Specifier
Panic attack is not a mental disorder and cannot be coded
Attacks can occur in the context of any anxiety disorder as well as other mental disorders and some medical conditions
When the presence of a panic attack is identified, it should be noted as a specifier
Symptoms are presented for the purpose of identifying a panic attack:
- palpitations, pounding heart, accelerated heart rate
- sweating, trembling, or shaking
- sensations of shortness of breath or smothering
- feelings of choking
- chest pain or discomfort
- nausea or abdominal distress
- feeling dizzy, unsteady, light headed, or faint
- chills or heat sensations
- paresthesias (numbness or tingling sensations)
- derealization or depersonalization
- fear of losing control or “going crazy”
- fear of dying
Agoraphobia
Marked fear or anxiety about 2 (or more) of the following situations:
- using public transportation
- being in open spaces
- being in enclosed spaces
- standing in line or in crowds
- being outside of the home alone
Fear and avoidance of such situations stems from thoughts that escape might be difficult or help might not be available
Fear or anxiety is out of proportion to the actual danger posed
Duration of 6 or more months
Generalized Anxiety Disorder
Excessive anxiety and worry (apprehension expectation), occurring more days than not for at least 6 months, about a number of events or activities
Difficult to control worry
Anxiety and worry is associated with 3 (or more) [1 in children] of following symptoms:
- restlessness or feeling of being on edge
- being easily fatigued
- difficulty concentrating or mind going blank
- irritability
- muscle tension
- sleep disturbance
