Indications for Amyl Nitrate
Cyanide poisoning
Amyl Nitrate’s mechanism of action
Convert hemoglobin to methemoglobin, which reacts with cyanide and chemically binds with it, preventing any toxic effects.
Contraindication
None in the emergency setting
Adverse effects of Amyl Nitrate
Headache, dizziness, weakness, increased ICP,
Shortness of breath, orthostatic hypotension,
Tachycardia, syncope, cyanosis
Precaution in using Amyl Nitrate
Increased hypotensive effects if pt. is on
Antihypertensive meds, phenothiazine’s,
ETOH, or beta blockers
Dose and administration of Amyl Nitrate
1 to 2 ampules crushed & inhaled for 30
Seconds each minute. After sodium nitrite
Admin, 30 seconds each 5 minutes
Pedi = 1 ampule, same rate
