emergency synchronised direct current (DC) cardioversion

Mx - haemodynamically stable patients with regular broad complex tachycardia
A
1/ vagal manœuvrés - carotid massage, valsalva manouvres
2/adenosine 6mg IV bolus

Mx - AF new onset <48hrs
A
rhythm control
LMWH followed by
- flecanide if no structual heart disease
- amiodarone if structural heart disease

Mx - AF onset >48hrs
A
rhythm control
metoprolol/bisoprolol/verapamil
or
digoxin if signs heart failure
+ anticoagulation

adenosine contraindications
A
heart transplant
central line
asthma - use verapamil instead
