- What are MOR/ASR’s?
Mandatory Occurrence Report and Air Safety Report. The ASR is a company scheme. The requirement to submit an MOR to the Authority is satisfied by filing an ASR. All ASR’s reviewed by BA who pass on/separate out those events not meeting the regulatory requirements.
- What constitutes an MOR?
“Any incident that endangers or could endanger the safety of the operation”.
- What is a ‘Reportable Incident’?
An occurrence other than an accident associated with the operation of an a/c affecting or which may affect the safety of the operation. A list of Reportable Incidents given in OMA 11A.
- How is an accident defined?
i) A person being fatally or seriously injured (excluding self-inflicted or by other persons or stowaways) as a result of ‘exposure’ to the aircraft.
ii) Damage/structural failure to a/c
iii) a/c missing
- Who should be notified immediately following an accident?
The BA reporting official – generally a person nominated by BA usually a senior BA official. Where no such official is present then the commander will take on those responsibilities.
- What sort of things should you as Reporting Official be doing after an accident?
Essentially preserve all evidence while tending to needs of survivors.
i) Safeguard FDR by pulling appropriate circuit breakers (entry in AML)
ii) Arrange immediate treatment to those requiring treatment and arrange welfare of crew/survivors.
iii) ASR/Witness statements etc
iv) Notify BA
- What do you do if an incident may generate media interest?
Call Ops Control Manager (OMA 11.7.6) by phone/Satcom as soon as practicable.
- When would crew be withdrawn from duty following an incident/accident?
Following any accident or incident that will be investigated on site by the State and/or BA Investigation Team.
- What do you need to do before you can resume duties having been withdrawn from them?
You are required to consult BAHS.
- When can you continue with your duties after clearance by BAHS?
Following re-instatment by Flight Manager subject to approval by Director of Flt Ops.
- What is an AirProx /Groundprox and what action(s) should you take?
Any situation in the opinion of pilot/ATC when the distance between a/c is such that its safety may have been compromised or avoidance action was taken. Must be reported immediately by R/T and confirmed by ASR.
- You have a structural exceedance and/or ACMS printout. What two things do you do?
All ACMS are transmitted automatically to Maintrol. If a commander has knowledge of any such incident he should file an ASR and make an entry in the AML.
- What weather conditions should you report immediately?
Any SIGMET conditions e.g. Icing/Turb/Volcanic Ash/Sandstorms/Mountain Wave
- What R/T terminology do you use to report SIGMET conditions you encounter?

- Define light/moderate turbulence?
Light > 5-15 kts in airspeed Moderate > 16-25 Kts
