MCI - Scope
- Applies to any incident where the treatment and transportation of multiple victims has the potential to
overwhelm the capabilities of the initial response resources and the Riverside County emergency medical system
*Incidents with 10 or more pts is considered an MCI
*Triage tags will be utilized on MCI and will be the only field PCR
*Strategies in this SOG can be utilized on an MPI as needed
MCI - (MPI side notes)
*Incidents of more than 1 but less than 10 pts that need to be transported are considered an MPI
*Med Comm will be established on every MPI
*MPI’s will have an electronic PCR for every pt.
MCI - General considerations
*Uses general concept of the 4 T’s
*Triage, Treat, Transport, Track
*Ribbon Triage: (colored Flagging), may be used for initial triage/sorting
*Triage tag will be utilized in treatment area or before transport
*Triage Tag number will be will be tracked on ICS-MC-306
MCI - Resource Ordering
*(1) Ambulance for every (2) Patients
*(1) Fire Apparatus for every (2) Ambulances (support for medical group only)
*(1) Air Ambulance for every (5) Immediate Patients, also consider for transport times greater than 30 min, pt access is delayed or ambulance access is delayed
MCI - Incident Communications
*COR Primary / COR Command for initial response and communication between IC and dispatch
*Operational TAC channel for tactical comms for units and personnel
*Pt transport TAC for the coord. of requesting ambulances and moving patients off the scene to an assigned hospital (typically used by the patient loading coordinator, ambulance coordinator, and med
comm)
MCI - Medical Communications (Med Comm)
*Once a clear picture of pts and incident complexities a paramedic will be assigned Med Comm. Only 1 Med Comm no matter the size of the incident
**Duties: Performs initial BHC to establish bed availability
**Communicates with Patient Loading Coordinator and Ambulance Coordinator
**Deploys position-specific clipboard tools and records on ICS-MC-308 and ICS-MC-306
MCI - Initial actions - 1st arriving company (Triage) - Company Officer
*Initial report on conditions
*Controls incoming resources
*Assigns the first arriving transport resource to the most critical patient and notes the destination
*Directs personnel on initial actions (Safety, Recon, Triage)
*Completes size up and declares MCI
*Establishes command if BC is not part of the initial dispatch
*Orders additional resources
*Establishes comms plan (Ensures 2 tactical frequencies assigned)
*Assigns additional resources to appropriate positions
*May be assigned Triage Unit Leader, Medical Group Supervisor, or any other ICS position based on incident need
MCI - Initial actions - 1st arriving company (Triage) - Engineer
*Completes a safety assessment
**Ensures traffic and/or crowd is controlled (separates bystanders from injured)
**Identifies and mitigates hazards to protect patients (deny entry, protection lines, electrical, etc.)
*Assists with triage if safety duties are completed
MCI - Initial actions - 1st arriving company (Triage) - Firefighters
*Perform initial recon of incident and reports number of patients and complexities (Fire/Extrication/Haz Mat)
*Deploys ribbon dispenser, Tactical Response Bag and begins triage
*Reports triage count and progress to Company Officer or Triage Unit Leader if established
*Re-assigned once triage is completed
MCI - Initial actions - 2nd arriving company (Treatment) - Company Officer
*Assigned Treatment Unit Leader
**Establishes treatment areas (as needed) and manages personnel and supplies
**Establishes treatment area managers as necessary
MCI - Initial actions - 2nd arriving company (Treatment) - Paramedic
*Assigned Patient Loading Coordinator (PLC)
**Deploys triage bag from MCI kit
**Deploys position-specific clipboard tools and records on ICS-MC-306
**Ensures all patients receive a triage tag prior to transport
*Remaining personnel assist with triage, treatment or other assignments
MCI - Initial actions - 3rd arriving company (Transport) - Company Officer
*Assigned Patient Transportation Unit Leader
**Ensures Med Comm is established and supported
**Determines the need to establish an Ambulance Coordinator (ensures ICS-MC-310 is used for tracking)
**Ensures routing and staging information is communicated to transport resources
**Dispatches ambulances to the loading area when requested by PLC
**Coordinates the establishment of Landing Zones with Medical Group Supervisor or IC
**Coordinates with the transport agency supervisor
