*Any incident where an arriving on scene company needs to enter and retrieve a victim who is located in a confined space and conditions meet the following:
**Potentially viable victim that can be seen from the exterior
**Entry can be made without removing SCBA
**A rigging system is not required for entry or removal of victim
**CGI reads less than 10% LEL
**Depth of entry no greater than 10’

Answer 2

*CO is initial action IC
*Directs crew to initial actions
*Report on conditions
*Requests add’l resources and stages them
*Requests tactical/command channels if needed
*Update on conditions and establishes/passes command
*Notify dispatch to contact public works and or relevant utility company

Question 3

Confined Space Line of Sight Rescue - Pre-entry phase

Answer 3

*Confirms atmosphere is less than 10 percent LEL
*Confirms lockout/tag out has been completed
*Personnel has donned appropriate PPE
*Personnel are equipped with basic victim removal equipment
*Signs Pre-Entry Check List prior to entry
*Assigns a medical group when additional resources arrive on scene

*I.C. documents atmospheric readings, entrant’s entry time, SCBA pressure, and signs signature box
*Entrant makes an entry with a tagline attached to his/her harness
*Rescue Attendant tends tag line and maintains communication with entrant
*Rescue Attendant and Back-up Entrant assist in hauling the victim out from outside the confined space
