What are the pertinent findings to suspect heart failure?
JVD to the angle of the jaw, S3, PMI is displace laterally, Edema
What testing will be needed for heart failure?
BNP is one of the best In the patient in class we should also try and rule out other things. So a CBC, CMP, TSH, and Lipid were also gotten.
What would be imaging study to get with a person with suspected HF?
Chest X Ray

It doesnt look completely normal but there isnt enough to be diagnostic of anything.
What is the US prevalance of HF?
5.8 million - tripled in the past decade
What is the clinical definition of heart failure?
Inability to adequately supply blood to metabolizing body tissues
What are some of the results of left sided heart failure?
Ischemia, hypertension, dilated cardiomyopathy, MI and restrictive cardiomyopathy.
Why is pulmonary congestion a main symptom of LHF?
Due to decreased forward perfusion leads to pulmonary edema then congestion
What is the role of the renin-angiotensin system in LHF?
LHF will cause decreased blood flow to the kidneys. This will eventually lead to activation of the renin-angiotensin system which will raise BP
What is the main treatment of LHF?
ACE inhibitor
What is the most common cause of RHF?
LHF
What are the clinical features of RHF?
JVD, hepatosplenomegaly - congested liver, dependent pitting edema.
What can lead to HF?
Chonic hypertension, MI, shunt
How can the nervous system exacerbate CHF?
The nervous system can recognize a lowering in the amount fluid present. This will act to increase sympathetic firing and increase TPR.
What will the Starling curve look like in a person with HF?
It will be shifted down due to decreased inotropic state
