In the event a badge is lost, stolen, or damaged you must:
- Notify supervisor reported immediately
- Complete a Survey Request (Form 41)
- Notify Badge Control Clerk and/or the I.D. Clerk within 24 hours
- Submit Form 40 (A Declaration of Lost Fire Department Issued Badge/Cap-Piece/Identification Card (Form 40)
