A
acute bacterial infection of the renal pelvis of the kidney

A
- Ascending infection from the lower urinary tract

Clinical features
A
- Fever
- Loin pain
- frequency, urgency and dysuria
- chills

Causes of Pyelonephritis
A
- Vesicouretral reflux - urine is able to move back up the urinary tract due to the valve dysfunction
- Bladder outflow obstruction - causes stasis of urine

Asymptomatic bacteruria
A
- Bacteria found in urine without any symptoms
- Only treated when found in pregnant women as there is 40% chance of progression into pyelonephritis

Urinalysis
A
- Nitrates - come bacteria convert into nitrates
- Leukocyte esterase enzyme released by wbc
- Leukocytes ( cloudy due to pyuria)
- Blood
- Proteins

A
- Mid stream urine - identifies organisms
