STATE the purpose of the EFM program

EFMP Cat 1
- Monitoring Purposes
- –EFM enrollees are those whose medical or educational condition requires monitoring by the EFM Program Manager but does not preclude the sponsor’s assignment to overseas/isolated duty stations, nor requires assignment near a major MTF, nor dictate homesteading
EFMP Cat 2
Pinpoint overseas assignment
—EFM enrollees are those whose medical or educational condition requires special placement in compatible geographic areas, pinpointing assignments in CONUS or overseas
EFMP Cat 3
No overseas assignment
- –EFM enrollees are exempt from overseas assignment
- –The medical and/or educational condition of the category of EFM precludes the assignment of the sponsor to overseas locations based on non-availability of medical and/or educational services
EFMP Cat 4
Assignments near major medical facilities INCONUS
- —-EFM enrollees require sponsor assignment near major medical facility (either military or civilian)
- —-These sites coincide with large fleet concentration locations
EFMP Cat 5
Homesteading
- – Stability
- – will not preclude the requirements for at sea/shore rotation
EFMP Cat 6
Temporary Category
—Enrollees require sponsor to enroll temporarily for a period of 6 months but no more than 12 months while treatment or diagnosis assessments are ongoing
Service member will submit updated application at the end of that time

Application process of EFMP
- Application forwarded to screening comitee
- Recommendations are forwarded to Chief, Navy Personnel Command (CNPC) for final disposition
STATE the responsibilities of the Service Member with regards to the EFMP
ID and enroll all family members with special needs
Review completed application
Update file every 3 years, 12 months prior to receiving orders, or as family needs change.
Maintain copy of the application
Dis-enroll from EFM and Inform EFM program manager (PERS-45) through CO when
Previously enrolled EFM has sufficiently recovered from impairment so that specialized medical care and or special educational services are no longer required
Divorced or legally separated
EFM dies
EFM no longer resides in home or is no longer DEERS eligible
