Antifibrinolytic Agent

TXA Action
A
Binds to plasminogen’s lysine receptor site and blocks the conversion of plasminogen to plasmin. The amount of plasmin is reduced thereby reducing the amount of fibrin(clot) breakdown

TXA Indications
A
- > 14 y/o or over 100 lbs(45.5 kgs).
- < 3 hrs post injury; ideal is < 1 hr post injury.
- SBP < 90
- Risk of significant bleeding, blood transfusion, or surgery.
- Multisystem trauma, patients with clinical concerns for bleeding in non-compressible areas.

TXA Contraindications
A
- > 3 hrs post injury.
- Age < 14 y/o or < 100 lbs (45.5 kgs).
- Isolated head injury.
- Isolated spinal shock.
- Non-blood loss injury.
- Isolated extremity hemorrhage where bleeding can be controlled and possibility of re-implantation exists.

TXA Dosage
A
Adult/Peds(>14 or 100 lbs) dose the same:
• IV/IO- 1 gram in 100mL NS over 10min.
