When can amniocentesis be done?
After 14 weeks gestation when uterus rises above the symphysis pubis and fluid amounts are adequate to get sample
The fluid from amniocentesis shoes the L/S ration, alpha fetoprotein, and desquameted fetal cells. What does each of those tell us?
L/s ratio: fetal lung maturity
AFP: Used for screening tool for NTD
Desquamated fetal cells: Allows for genetic testing
What is important about amniocentesis and the lab?
Sunlight can affect the cells. Immediately after obtaining fluid tube should be kept away from light source!! Must take the specimen in a brown paper bag when transporting to lab
What is surfactant?
A substance that reduces the surface tension of pulmonary fluids to allow gas exchange in the alveoli
*basically keeps lungs from sticking together
Why is amniocentesis done in early preg?
To detect chromosomal abnormalities
Why is amniocentesis done in late preg?
To determine fetal lung maturity with LS ratio to detect amount of surfactant production in fetal lungs
What are some complications of amniocentesis?
- Infection
- Injury
- Leakage of AF
- Preg loss
- Mom hemorrhage
- Rh iso
- AF embolism
- Fetal death
Does mom need to empty bladder or have full bladder for amniocentesis?
EMPTY
How often are we monitoring fetus during amniocentesis? How do we monitor?
EFM; monitor before procedure and at least 1 hour post
When is CVS done and where?
10-13 weeks; sample taken from edge of placenta
How much blood is withdrawn from PUBS?
1-4 mL
Why is Kleihauer Betke test done after PUBS?
To see if any maternal-fetal cross contamination happened at cord site
