substance use and addictions Flashcards
what is a harmful substance under icd10
actual damage caused to mental/physical health of user in absence of diagnosis of dependence syndrome
what is dependence syndrome according to icd10
- strong desire/sense of compulsion to take substance
- difficulties in controlling substance taking behaviour in terms of it onset, termination, or levels of use
- physiological withdrawal when substance has been stopped
- evidence of tolerance: need to start taking more
- persisting despite knowing harm
(need 3 to be dependent)
what is addiction
compulsive drug use despite harmful consequences, characterised by inability to stop using a drug, failure to meet work, social or fam obligations, tolerance and withdrawal
what does dependence mean in biology/pharmacology
refers to physical adaptation to a substance
- tolerance/withdrawal
- can be dependent but not addicted
what are some behavioural addictions
gambling disorder
internet gaming disorder
what is gambling disorder
similarities in aetiology, neurobiology and treatment approaches and comorbidities with substance dependence.
why do some individuals drink?
alcohol alters balance between brains inhibitory and excitatory systems
what happens if you drink alcohol acutely
blocks excitatory system - impaired memory (alcoholic blackouts) boosts inhibitory system - anxiolysis - sedation
what does chronic alcohol exposure result in
neuroadaptations so gaba and glutamate remain in balance in presence of alcohol
- upregulation of excitatory system
- reduced function in inhibitory system (tolerance)
what is the treatment to help with alcohol withdrawal?
treat with benzodiazepines to boost gaba function
where is dopamine found
in part of brain called ventral striatum
what can increased dopamine levels in brain
natural rewards e.g food, sex
drugs of abuse can also increase dopamine
how does cocaine and amphetamine affect domaine levels
cocaine and amphet block reuptake of dopamine
amphetamine enhances release of dopamine
change from voluntary drug use to more habitual and compulsive drug use involves transition from….
prefrontal to striatal control over drug taking i.e prefrontal control is diminished with greater striatul reward drive
what is positive reinforcement controlled by
dopaminergic mesolimbic system
