  6. Bears the signature and license number of the veterinarian issuing the certificate

A

Dog is at least 6 months, dog is vaccinated not more than 12 months before arrival with DHLPP, dog is in good health, and dog is vaccinated for rabies. Provided that such person importing dog for resale to research submits satisfactory evidence to Animal Care at time of application for an import permit that the specific provisions would interfere with the dog’s use in such research in accordance with the research proposal, and the proposal has been approved and reviewed by the facility IACUC.

The provisions for age, health status, and vaccination also do not apply to import of dogs to the States for what? In what circumstances?

A

For veterinary treatment by a licensed vet and subsequent resale. Provided that original health certificate states that the dog is in need of vet treatment that cannot be obtained in country of export and states name and address of licensed vet in US who intends to provide such treatment AND person who imports dog completes a vet treatment agreement with Animal Care at time of application for an import permit and confines animal until the conditions specified in the agreement are met. Person importing dog shall bear expenses of treatment and confinement.

Dogs imported into Hawaii from the British Isles, Australia, Guam, or New Zealand are exempt from what provision?

A

Need to be at least 6 months. Dog cannot be transported out of Hawaii for purposes of resale at less than 6 months of age.
