Each dealer or exhibitor must maintain records that include what information? What groups are excluded?
- Name and address of person from whom the dog or cat was acquired.
- USDA license or registration number of the person is licensed
- Vehicle license number and State, and driver’s license number (or photo ID Card) and State of the person if not licensed
- Name and address to the person to whom a dog or cat was sold and that person’s license or registration number if they are licensed/registered
- Date the dog or cat was acquired or disposed
- Official USDA tag number or tattoo
- Description of dog or cat including species and breed or type, sex, date of birth or approximate age, color and any distinctive markings
- Method of transportation, including name of initial carrier or intermediate handler, or if privately owned vehicle used, the name of the owner of the privately owned vehicle
- Date and method of disposal
Must include any offspring born of the animal while under their possession/control
Operators of auction sales and brokers to whom animals are consigned are exempted.
Which forms must a dealer or exhibitor use to record the required information regarding dogs and cats? Can the dealer request an exemption? Can the dealer/exhibitor request a hearing if their exemption is denied?

Can request an exemption if using a computerized recordkeeping system and believe these forms are unsuitable and describes the computerized record-keeping system the person would use in lieu. Can request a hearing, but denial shall remain in effect until the final legal decision has been rendered.
One copy of the record must accompany any shipment of dog of cats, but what information can be excluded?
Information that indicates the source and date of acquisition of a dog or cat.
What records must a dealer or exhibitor, excluding an operator of an auction sale and broker, keep regarding animals other than dogs and cats?
Records shall include offspring born or hatched, to the extent that identification/counting offspring can be carried out without unduly disturbing nesting or rearing activities

Operators of auction sales or brokers must retain what records, even if a fee or commission was not charged?
- Name and address of person who owned and consigned animal for sale
- Name and address of the buyer or consignee
- USDA license or registration number of person selling or buying
- Vehicle license number and State, and driver’s/photo ID and State if they are not licensed
- Date of consignment
- Official USDA tag number or tattoo
- Description of animal including species and the breed/type, sex (only if readily determinable for bird), date of birth or hatch or approximate age or developmental stage, color and any distinctive markings
- Auction sales number or records number assigned to animal
Who must receive a copy of the record following an auction/broker? What can be excluded from the record given to the buyer?
Consignor, purchaser, and one retained by the operator or broker. Buyer record does not need to include source and date of consignment of animal.
In connection with all live animals accepted for shipment on a COD basis or other practice in which the cost of an animal or transport is to be paid and collected upon delivery to consignee, what record should be maintained?
The accepting carrier or intermediate handler shall keep and maintain a copy of the consignor’s written guarantee for the payment, including the return transportation charges and an amount sufficient to reimburse carrier for out-of-pocket expenses for care.
What record should the carrier or intermediate handler keep regarding attempts to contact the consignee?
Copy of the shipping document containing the time, date, and method of each attempted notification and the final notification to the consignee and the name of the person notifying the consignee.
What must the carrier or intermediate handler have for all live dogs, cats, or NHPs delivered for transportation?
Copy of the health certificate
What must a health certificate state?
- Licensed vet inspected dog, cat, or NHP on a specified date not more than 10 days prior to the delivery of the animal for transport
- When inspected, the animal appeared to be free of any infectious disease of physical abnormality that would endanger the animal or other animals or endanger public health
Who can provide exemptions to the requirement for a health certificate for research facilities?
Secretary

How often must a carrier or intermediate handler receiving an animal at a destination on a COD basis notify the consignee? What information must be recorded?
At least once every 6 hours for a period of 24 hours after arrival of the animal at the terminal cargo facility. Must record the time, date, and method of each attempted notification and the final notification to the consignee, and the name of the person notifying the consignee, on the shipping document and on the copy of the shipping document accompanying the COD shipment.
How long does the cosignee have to respond to a COD shipment? What occurs then?
24 hours. Carrier/intermediate handler return the animal to the consignor, or to whomever the consignor has designated, on the next practical available transportation, in accordance with the written agreement and notify the consignor.
