A
  • palpable lump
  • characteristically >5cm
  • painful
  • grows rapidly
  • below superficial fascia

A

imaging: CT, MRI, FDG-PET, US

  • 65% extremity and torso
  • 15% retroperitoneal
  • 10% head and neck

Radiation therapy for soft tissue cancer

  • to spare limbs
  • pre- or post-op.
  • IMRT
  • brachytherapy
  • Ewing’s sarcoma + rhabdomiyosarcoma are radiosensitive

Pharmacological therapy for soft tissue cancer

A

monotherapy:
- doxorubicin
- ifosfamide

combined therapy:
- CYVADIC scheme
- MAID scheme
- VIP (vepesin, ifosfamide, cisplatin)

  • angiosarcoma: taxanes + gemcitabine
  • leiomyosarcomas: gemcitabine (second/third line chemo: dacarbazine)
  • brain/uterine: temzolomide
  • peripheral nerve sheath tumors: etoposide + carboplatin
